The purpose of this study was to evaluate the effect of the lateral approach to the lateral ankle ligament complex by examining 2 aspects of the preoperative and postoperative ankle joint laxity, pain, range of motion (ROM), and strength. Two hundred ankles in 101 consecutive patients undergoing surgical repair of a lateral ankle ligament complex injury were evaluated. Patients were divided into 2 groups according to the surgical approach, and the mean ankle laxity was measured preoperatively and at the 3-month follow-up. Patients' assessment of the pain using a visual analog scale (VAS) was evaluated preoperatively and postoperatively at the 3-month follow-up. The mean anterior drawer and posterior drawer tests were negative in both groups before surgery and remained negative in the anterior drawer test at the 3-month follow-up (p .05). The overall strength of the ankle joint was significantly better in the lateral incision group (p =.03). The lateral incision approach has advantages for repair of lateral ankle ligament complex injuries compared with the standard medial incision approach.
